Overview
A late applicant is when the dependent is added to the member's plan after 31 days of eligibility.

Adding Dependent
- Send the Change form, showing the dependents requested information \\superserver.unipharm.local\Payroll\Healthsource\Forms
- The Carrier will issue the appropriate Evidence of Insurability directly to the member. For privacy reasons this cannot be sent to unipHARM, they will mail this to the member for completion. Once a decision is made, the member will be notified. Only at that time will the Carrier, advise Healthsource of the decision & the effective date this would follow.
