If the new applicant does not yet have MSP (Medical Service Plan of BC) confirmation, Equitable Life will have to wait for the MSP confirmation date and will use the MSP effective date as the effective date on the group plan. MSP usually takes some time to process the application and will back date to the original request date. If Equitable Life is advised after 31 days from the MSP effective date, the employee will be considered a late applicant. You must email Equitable Life at email me and explain the situation and request an exception. If granted, they will waive the late status and not require the Statement of Health for Group Insurance form [[unc\\\superserver\forms\formsgeneral\equitable life\forms\Supplementary Health Benefits Claim Form #466(2012_07_13).pdf|Supplementary Health Benefits Claim Form}} to be filled out.
