Overview
After employee injured at work, First Aid Attendants have to complete First Aid Record and forward the copies to payroll ASAP. Payroll will complete WCB Form-7 online, process payroll and file. Employer are required to report injuries within three days of the injury to avoid penalties and ensure WCB has our information to make decisions about the claim and any benefits WCB can offer. Before complete WCB Form-7 online, please make sure have following information ready. - First Aid Record - Employee's SIN#, birthdate, address, home phone#, physician's name, address, phone# and visit date - Employee's work schedule, rate of pay and, if possible, his or her total earning over the last 12 weeks (3 months)
This information is only required if the employee has missed time from work due to the injury since the day of the injury
- Employee's days (or shifts) missed due to the injury, if any - Whether transitional work and /or a modified work schedule to support a safe return to work are available and have been offered

- WCB will take few days to general claim files
