Overview

All shareholder store LTD communications should be directed to the pharmacy owner, not uniPHARM head office. As of March 2017, changes were made to ensure that Equitable Life made the necessary changes to change the primary contact for all policies with the exception of 97752 to the pharmacy owner and not uniPHARM head office. Instead of taking the information and forwarding it to Equitable Life, please have the pharmacy owner contact Equitable Life directly.
