Accounting Finance:Equitable Life-Fair Pharmacare
Overview
Equitable Life requires a copy of the Confirmation of Fair PharmaCare Assistance letter. Copies of the letter can be mailed to Equitable Life (attention Group Administration), One Westmount Road North, PO Box 1603 Stn. Waterloo, ON N2J 4C7 or emailed to Equitable Life Group Benefits or via fax to 1-888-878-7747. Plan Members who are already registered and have provided Equitable Life/uniPHARM with the Confirmation of Fair PharmaCare Assistance letter will not have to do anything.
- Note - uniPHARM will be unable to make any Fair Pharmacare changes on the Equitable Life web-site; therefore, all forms are forward to Equitable at the above address for processing.
Fair PharmaCare Registration
To inquire about the status of your Fair PharmaCare registration, use the link below and enter either your registration number or your personal health number. Please remember that PharmaCare requires your consent to verify your income with the Canada Revenue Agency (CRA) and to confirm your level of financial assistance under Fair PharmaCare. If you register by telephone or on-line, a consent form will be mailed to you. You will need to complete and sign the consent form, and return it to PharmaCare as soon as possible to ensure your temporary coverage continues. If you register by mail, estimated to take 6-8 weeks, sign the consent and registration form, and return it to PharmaCare.
If you do not return your signed consent form, Fair PharmaCare is unable to determine your appropriate level of coverage. Without confirmation of your income, your level of coverage would be set at the default level of a $10,000 deductible per family.
